Plan to arrive at least 15 minutes before your appointment so that we
may process your paperwork and answer any questions you have. You are
responsible for bringing your most recent insurance card(s) and a list
of all your current prescription and over-the-counter medications,
including dosages. All patients are required to sign in at the reception desk upon arrival. Your co-pay will be due when you check out. Eating and drinking are not allowed in the waiting area or in the exam rooms. Cell phones and pagers need to be turned off or put on silent mode while in the exam rooms. Smoking is not permitted inside the building; you may smoke outside in designated areas only. Abusive language or threatening or violent behavior will not be tolerated in the office.
