An appointment is required for all services provided. You may schedule
an appointment by calling our office or you may request an appointment
online (click "Create An Account" above, fill out the required information, then click
"Appointment Requests"). We will contact you forty-eight hours before your appointment to confirm the date and time. If you need to cancel or reschedule an appointment, we request that you notify us at least 24 hours in advance. This will allow us to offer the time slot to other patients who need to be seen.
